肋横突上韧带（肋横突前韧带）下方附着于肋颈上缘的锐嵴，斜向上外方走行，连接至紧邻其上方的横突下缘。其前方与肋间血管和神经相邻；其内侧缘增厚且游离，围成一孔，肋间血管和神经的后支由此穿过；其外侧缘与一薄层腱膜相延续，该腱膜覆盖于肋间外肌表面。
第一肋无肋横突前韧带。一束纤维，称为腰肋韧带，与各肋横突前韧带相延续，连接第十二肋颈与第一腰椎椎骨横突基部；该韧带仅为腰背筋膜后层的增厚部分。
