颈总动脉在长度及起源方式上有所不同：
- 右颈总动脉起自无名动脉（头臂干）的分叉处，位于胸锁关节后方，全程走行于颈部。
- 左颈总动脉起自主动脉弓最高部，位于无名动脉的左侧及其后方平面，因此由胸部段和颈部段两部分组成。左颈总动脉的胸部段自主动脉弓向上穿过上纵隔，至左胸锁关节水平，与颈部段相延续。
颈总动脉在甲状软骨上缘处分为颈外动脉和颈内动脉，约位于第四颈椎水平。
