钩骨的掌侧面在其下方和尺侧，有一弯曲的钩状突起，称为钩突，朝前方和外侧方延伸。此突起以其尖端为附着处，连接腕横韧带和尺侧腕屈肌；其内侧面附着短屈肌和小指对掌肌；其外侧面有一沟槽，供屈肌肌腱通过进入手掌。它是腕骨前面四个隆起之一，腕横韧带即附着于这四个隆起；其余三个分别为内侧的豌豆骨、大多角骨的斜嵴以及外侧的舟骨结节。
