钩骨的掌侧面在其下方及尺侧，有一弯曲的钩状突起，称为钩突，朝前方及外侧方延伸。此突起以其尖端附着于腕横韧带和尺侧腕屈肌；以其内侧面附着于小指短屈肌和小指对掌肌；其外侧面有一沟槽，供屈肌腱经过进入手掌。它是腕骨前面四个隆起之一，腕横韧带附着于这四个隆起之上；其余三个分别为：内侧的豌豆骨、大多角骨的斜嵴以及外侧的舟骨结节。
