项韧带是一种纤维膜，在颈部，代表下位椎骨的棘上韧带。
它从枕外隆凸和项中线延伸至第七颈椎棘突。
从其前缘发出一纤维薄板，附着于寰椎后结节和颈椎棘突，并在颈部两侧肌肉之间形成一个间隔。在人类中，它仅是一条重要弹性韧带的退化痕迹，而在某些低等动物中，该韧带起到支撑头部重量的作用。
