小脑半球第三小叶（又称小叶III[H]）是小脑半球前叶的组成部分。

根据Larsell的划分，前叶包含小脑半球的五个小叶（I至V[H]）。这些小叶的形成源于多条横行裂隙的出现，这些裂隙在内外侧平面上贯穿小脑半球与蚓部。小脑半球第三小叶与第二小叶（II[H]）共同被称为"ala lobuli centralis"（依据经典命名法）。
