胃左静脉从胃的两个面接收属支；它沿胃小弯从右向左走行，位于小网膜两层之间，至胃的食管开口处，在此接受数条食管静脉。随后转向后方，从左向右经网膜囊后方走行，最终汇入门静脉。
胃左静脉的终止方式存在变异：可汇入门静脉（占46–65%的病例），或汇入脾静脉（占30–33%的病例）。胃左静脉走行于肝总动脉或脾动脉的前方或后方，这可影响其引流方式。
在古典命名法中，胃右静脉与胃左静脉共同构成冠状静脉。
