Definition
The cremaster muscle (from the Greek "kremaster," meaning "suspender") is a thin layer of striated skeletal muscle that descends along the spermatic cord and surrounds the testis in males. It originates from the internal oblique and transversus abdominis muscles as the testes descend through the inguinal canal during development.
Anatomy and Innervation
The cremaster muscle is innervated by the genital branch of the genitofemoral nerve, which arises from the L1 and L2 spinal nerve roots. This nerve provides motor fibers to the cremaster and sensory innervation to the scrotum and upper thigh. The nerve penetrates the psoas muscle at the level of L3L4, descends along its medial border, and enters the inguinal canal through the internal inguinal ring alongside the spermatic cord.
Functions
Testicular thermoregulation: The primary role of the cremaster muscle is to regulate testicular temperature by elevating or lowering the testes relative to the body. Contraction pulls the testis upward toward the superficial inguinal ring, reducing heat loss; relaxation allows the testis to descend, facilitating heat dissipation.
Support of spermatic cord veins: During increases in intra-abdominal pressure (e.g., straining, coughing), the cremaster contracts to support the cord veins and prevent venous reflux from abdominal veins into the pampiniform plexus.
Testicular descent: During fetal development, the cremaster muscle plays a role in inguinoscrotal testicular descent, guided by androgens and calcitonin gene-related peptide (CGRP) released from the genitofemoral nerve.
Sexual reflexes: The muscle contracts during erection (the "peno-cremasteric reflex"), which may facilitate testicular elevation and expression of testicular secretions.
Unique Properties
Unlike most striated muscles, the cremaster muscle largely lacks voluntary control and functions primarily through reflexes. It also has unique contractile properties: elevated temperature paradoxically increases its contractility (opposite to other skeletal muscles), which may reflect its specialized thermoregulatory role. Additionally, it contains multiple motor end plates and matures more slowly than other body muscles, with persistence of immature myogenic proteins similar to those found in cardiac muscle, allowing rhythmic contraction.
Clinical Significance
The cremasteric reflex elicited by stroking the inner thigh, resulting in ipsilateral testicular elevation is an important clinical sign. Its absence may suggest testicular torsion, upper or lower motor neuron lesions, genitofemoral nerve injury, or L1L2 radiculopathy. However, the reflex can be absent in healthy older men and its reliability as a localizing sign is debated. The cremaster muscle is also a prominent surgical landmark during inguinal hernia repair and orchiopexy, and the mouse cremaster muscle preparation is widely used in microvascular research for intravital imaging of the microcirculation.
