后穿质位于大脑脚之间的凹陷区域（称为脚间窝），由一层灰质构成，该灰质被细小的纹状血管穿透（包括大脑后动脉的分支：后内侧中央动脉和丘脑穿动脉）。这些血管起源于Willis环，穿过后穿质深入脑组织。这一排列为重要的皮质下脑区提供血液供应，包括底节、内囊和丘脑。
其上部参与构成第三脑室的底壁。
