The anterior wall of stomach forms the front-facing surface of the stomach and is completely covered by visceral peritoneum (serosa). Its orientation varies according to the degree of gastric distension. In contracted state, the gastric surfaces are directed predominantly upward and downward; however, as the stomach becomes distended, these surfaces rotate to assume a more anterior and posterior orientation. Consequently, the surfaces are often described as the anterosuperior and posteroinferior surfaces.

Relations of anterior wall of stomach

  • The left half of the anterior wall of stomach is related to the diaphragm, which separates it from the base of the left lung, the pericardium, and the seventh, eighth, and ninth ribs, and intercostal spaces on the left side.

  • The right half of the anterior wall of stomach is related to the left and quadrate lobes of the liver, and where not covered by the liver, to the anterior abdominal wall. In the empty stomach, the transverse colon may lie in contact with the lower part of this surface. The entire anterior wall is invested by visceral peritoneum.

Vasculature

The blood supply of anterior wall of stomach is derived primarily from branches of the left and right gastric arteries along the lesser curvature and the left and right gastro-omental arteries along the greater curvature. Venous drainage follows the corresponding arteries and ultimately drains into the portal venous system.
