Tibial pilon

Tibial pilon

Definition

Antoine Micheau

The tibial pilon refers to the distal articular segment of the tibia, specifically the weight-bearing surface sometimes termed as the “tibial plafond”. This region forms the roof of the ankle joint and articulates with the talus.

This term is not listed in Terminologia Anatomica 2 and may be analagous to the “Inferior articular surface of tibia” butthe term "pilon" is used in orthopaedic trauma to describe fractures involving this area, typically resulting from high-energy axial compression mechanisms, such as falls from height or motor vehicle accidents, where the talus is driven into the tibial plafond, causing comminution and displacement of the articular surface.

Pilon fractures are characterized by intra-articular involvement, significant comminution, and frequent soft tissue compromise. These injuries are considered potentially limb-threatening due to the risk of severe soft tissue damage and complications such as infection, nonunion, and posttraumatic arthritis.
