前臂后皮神经是桡神经的感觉支，起源于桡神经穿行桡神经沟时。其起始点位于臂外侧下皮神经起始点的远侧，但在肱骨外上髁近侧的上臂远段范围内。该神经发出后穿过肱三头肌外侧头，随后穿透深筋膜，在外上髁近侧约6厘米处浅出皮下。

该神经发出一支髁上支，并向远侧下行，从外上髁前方越过。随后经前臂近段肱桡肌与桡侧腕长伸肌之间的间隙走行。此后沿前臂后面下行，最终在其远端附近与前臂外侧皮神经的背侧支汇合。
