The posterior superior pancreaticoduodenal vein is a vein that drains the posterior aspect of the pancreatic head and duodenum. It frequently runs postero-inferior to the common bile duct and varying from authors, does not accompany an artery, or may accompany the corresponding posterior inferior pancreaticoduodenal artery. The posterior superior pancreaticoduodenal vein drains into the portal vein above the splenic vein-portal vein confluence, with a mean distance of approximately 0.3 cm from this confluence.

Variations in its anatomy are common, with some cases showing the posterior superior pancreaticoduodenal vein crossing the ventral side of the common bile duct.
