The calcaneofibular ligament is one of the three primary components oof the lateral collateral ligament of ankle (lateral ankle ligament complex), along with the anterior talofibular ligament (ATFL) and the posterior talofibular ligament (PTFL). It is a cord-like or flat, fanning structure that originates from the anterior border of the distal fibula, specifically just proximal to the tip of the lateral malleolus, and inserts onto the lateral wall of the calcaneus, posterior and superior to the fibular trochlea.

The ligament measures between 18.5 and 35.8 mm in length and 4.6 to 7.6 mm in width, though there is significant anatomical variation. Morphological variants include single bundle, Y-shaped, V-shaped, and multi-banded forms, with the single bundle and Y-shaped double bundle being most common.The fibular origin of the CFL is often confluent with the ATFL and PTFL footprints, forming a continuous attachment at the medial aspect of the fibula. The calcaneal insertion is a single footprint located approximately 12–27 mm posterior and superior to the peroneal tubercle.
