The posterior medial segment (segment VIII) is the superior subsegment of the anterior segment (right medial division), situated to the right of the right hepatic veins, above the portal plane.

Note: The Terminologia Anatomica 1&2 is remarkably not at all clear for the denomination of segments of right liver, and these terminologies are not justified in official terminologies without any source cited for these terms. The use of the adjectives "anterior" for segment V and VI and "posterior" for segments VIII and VII is very strange and doesn't match with the anatomical orientation of these segments.
