Sinuatrial nodal branch of right coronary artery

Ramus nodi sinuatrialis arteriae coronariae dextrae

  • Synonym: SA node artery
  • Related terms: Sinuatrial nodal branch

Definition

Antoine Micheau

The sinuatrial (SA) nodal branch of the right coronary artery is a small arterial branch that arises from the right coronary artery (RCA) and supplies blood to the sinoatrial node, the heart's primary pacemaker. Here is a comprehensive definition:

Origin and Prevalence

The SA nodal artery most commonly originates from the right coronary artery (in approximately 5565% of individuals), typically arising as one of the first branches near the origin of the RCA from the right aortic sinus. In the remaining cases, it arises from the left circumflex artery (about 35-45%) or, rarely, from both coronary arteries.

Course

After its origin, the SA nodal branch courses posteriorly and superiorly, passing between the right auricular appendage and the aortic root. It encircles the superior vena cava (SVC) either anteriorly, posteriorly, or forming a complete loop around it before reaching the sinoatrial node, which is located at the junction of the SVC and the right atrium (at the sulcus terminalis/crista terminalis region).

Territory of Supply

  • The sinoatrial node (the dominant cardiac pacemaker)

  • Adjacent right atrial myocardium

  • Portions of the interatrial septum (in some anatomical variants)

  • Part of the left atrial wall (when the artery encircles the SVC)

Clinical Significance

  • Acute coronary syndromes: Occlusion of the proximal RCA can compromise the SA nodal branch, leading to sinus node dysfunction, including sinus bradycardia, sinus arrest, or sinoatrial exit block. This is particularly relevant in inferior myocardial infarctions.

  • Cardiac surgery: The SA nodal artery is at risk during procedures involving the right atrium, SVC cannulation, or atrial septal defect repair. Inadvertent injury may result in postoperative sinus node dysfunction.

  • Anatomical variability: Knowledge of the SA nodal artery's origin and course is essential for interventional cardiologists and cardiac surgeons to avoid iatrogenic injury.

Anatomical Classification

The SA nodal branch is classified as an atrial branch of the coronary arterial system. It is one of the most clinically important atrial arteries due to its role in perfusing the cardiac conduction system.
