The middle cardiac vein (vena cardiaca media) begins near the apex, runs in the subsinuosal interventricular groove alongside the corresponding branch of the cardiac artery, the subsinosal interventricular branch.

In carnivores (like in humans), the middle cardiac vein ends in the coronary sinus near the atrial ostium.

In horses, the middle cardiac vein drains directly in the righ atrium.

The middle cardiac vein drains deoxygenated blood from the interventricular septum (septal branches) and adjacent ventricular walls (right and left ventricular branches).
