髌下脂肪垫（IFP；Hoffa脂肪垫）是一种囊内滑膜外结构，位于髌骨下缘、股骨髁、胫骨平台与髌腱之间的间隙中。
在解剖学上，髌下脂肪垫由白色脂肪组织组成，脂肪组织被细薄的结缔组织间隔分隔为小叶状结构。该结构血管和神经分布丰富，含有大量血管与神经。髌下脂肪垫附着于膝关节内的多个结构，包括髌骨下极、经黏膜韧带连接的股骨髁间切迹、髌腱近端、两侧半月板，以及经半月板胫骨韧带连接的胫骨前方。
髌下脂肪垫包含以下结构：
- 一个具有上方垂直裂隙的中央体
- 两个上方延伸部：外上方延伸部和内上方延伸部，两者环绕髌骨，可在MRI上显示，并可分别在内侧和外侧识别。这些延伸部与髌下脂肪垫的动态特性使其能够在膝关节运动过程中发生形变，在膝关节屈曲时向上方和后方移位。
- 内侧延伸部和外侧延伸部
在组织学上，髌下脂肪垫靠近髌腱处由较大的脂肪小叶构成，深层部分则由较小的小叶组成，浅层小叶在膝关节屈伸运动过程中表现出明显的体积变化。这种结构排列提示髌下脂肪垫在吸收压力变化及参与膝关节运动学方面具有重要作用。
