脊髓后中间沟是位于脊髓背侧（或后侧）面的一条附加沟，介于后正中沟与右室后支之间。

脊髓后中间沟将背侧感觉上行柱分为两半——内侧为薄束，外侧为楔束。

薄束代表背柱内侧丘系通路的第一级神经元，负责传导来自下肢（脊髓节段T6及以下）的振动觉、本体感觉和两点辨别觉等感觉信息，并沿脊髓背侧和延髓上行，与薄束核形成突触联系。相反，楔束同样构成背柱通路的一部分，传导来自上肢（脊髓节段T6及以上）的相同感觉信息，并沿延髓背侧——位于薄束外侧——上行，最终与楔束核形成突触联系。

请注意，由于T6以下不存在楔束，因此脊髓后中间沟在脊髓下半部分无法观察到。
