Muhammad A. Javaid

The submucosa of pharynx is a connective tissue layer situated between the mucosal lining internally and the muscular wall externally. It consists of loose fibroelastic connective tissue containing blood vessels, lymphatics, nerves, mucous and seromucous pharyngeal glands, and aggregates of lymphoid tissue. Functionally, it supports the overlying mucosa, permits its mobility, and serves as a conduit for neurovascular structures supplying the pharyngeal wall.

Composition of submucosa

The composition of the submucosa varies regionally according to pharyngeal function.

  • In the nasopharynx, it is particularly rich in mucous glands and lymphoid tissue associated with the pharyngeal tonsil, reflecting the respiratory role of this region.

  • In the oropharynx and laryngopharynx, the submucosa supports the non-keratinised stratified squamous epithelium and contributes to lubrication during swallowing.

Pharyngobasilar fascia

Superiorly, where the muscular wall of the pharynx is deficient because the superior constrictor muscle does not fully reach the skull base, the submucosal connective tissue becomes condensed into a dense fibroelastic layer known as the pharyngobasilar fascia. This fascia lies between the mucosa/submucosa internally and the pharyngeal muscles externally and provides important structural support to the upper pharyngeal wall by anchoring it to the base of the skull.

Relationship between submucosa of pharynx and pharyngobasilar fascia

Thus, the submucosa of pharynx represents the general connective tissue layer of the pharyngeal wall, whereas the pharyngobasilar fascia is a specialised thickened reinforcement of this layer. The distinction is particularly important in the superior pharynx, where the fascia compensates for the relative deficiency of muscle fibres and helps maintain the integrity of the pharyngeal wall.
