岩小神经沟位于三叉神经节压迹和岩大神经沟的外侧下方。自此处沿颞骨岩部前面向后外侧方向延伸，终止于岩小神经裂孔。
岩小神经本身起自岩骨中耳腔内的鼓室丛，由其裂孔处穿出。该裂孔位于岩大神经裂孔的稍下方。岩小神经随后沿向内侧走行，朝向卵圆孔，在整个行程中始终位于岩大神经走行路径的下方及外侧。最终，岩小神经穿入卵圆孔，进入颞下窝区域。了解岩小神经的解剖走行及位置至关重要，因为该神经传导负责支配腮腺分泌的副交感神经纤维。
