股骨远端解剖轴是用于评估股骨成角畸形并为矫正提供指导的解剖标志。它在矢状面上通过股骨全长的内外侧位X线片来确定。当内侧髁与外侧髁完全重叠时，认为体位可接受。
首先在小转子与股骨滑车近端部分之间画一条线，该线将股骨等分为4段。在远端段内，沿垂直于股骨骨干的方向画两条线。股骨远端解剖轴连接这两条线的中点。
Peterson等人（2020）提出了股骨远端解剖轴的另一种定义：他们将其定义为垂直于股骨远端关节定向线中点的轴线（依据其自身的定义）。
