Definition
The superomedial extension of the infrapatellar fat pad refers to a distinct anatomical projection of the infrapatellar fat pad (IFP) that extends superiorly and medially from its main central body, attaching to the superior border of the patella and the anterior aspect of the medial patellar retinaculum. This extension is consistently present in the majority of knees and is typically larger than the corresponding superolateral extension. It forms a pedicle-like structure positioned between the patella and the medial trochlea, and in some cases, may loop around the superior border of the patella.
The superomedial extension is considered part of the dynamic and mobile architecture of the IFP, which deforms and displaces during knee motion, potentially influencing patellar tracking and anterior knee pain syndromes.
