The proximal tendon of the medial head of the gastrocnemius muscle originates from the posterior aspect of the medial femoral condyle, specifically from an area just proximal and posterior to the medial epicondyle, and adjacent to the gastrocnemius tubercle.

The tendon is broad and flat at its origin, forming a strong aponeurotic structure that transitions into muscle fibers distally.

Unlike the lateral head of gastrocnemius, the medial head’s proximal tendon is typically free from major neurovascular structures, which is clinically relevant for surgical approaches and tendon release procedures.
