The anterior root of the medial meniscus is a fibrous structure that anchors the anterior horn of the medial meniscus to the tibial plateau into the anterior intercondylar area of the tibia, just anterior and medial to the tibial attachment of the anterior cruciate ligament (ACL).

Histologically, the attachment is a classical enthesis, with dense central fibers inserting directly into cortical bone, and peripheral supplemental fibers contributing to the overall footprint.

The anterior root typically appears as a single hypointense fiber bundle on MRI, distinguishing it from the more complex fiber arrangements of other meniscal roots.
