The posterior horn of the medial meniscus is the thickened, posterior segment of the medial meniscus in the knee, anchored to the posterior intercondylar area of the tibia. It is stabilized by the meniscocapsular and meniscotibial ligaments, which attach to its posterosuperior and posteroinferior edges, respectively.

This region is highly relevant clinically due to its propensity for ramp lesions—tears at the meniscocapsular or meniscotibial junction—especially in association with anterior cruciate ligament injuries.
