The tendon sheath of the flexor digitorum longus envelops the flexor digitorum longus tendon as it passes through the tarsal tunnel and into the plantar aspect of the foot. The sheath consists of two main layers: an outer fibrous layer and an inner synovial layer. The synovial layer produces synovial fluid, which facilitates low-friction gliding of the tendon during movement. The sheath begins proximally near the medial malleolus and extends distally as the tendon courses beneath the sustentaculum tali, through the plantar foot, and into the toes.
