The brainstem is the caudal portion of the encephalon that connects the forebrain to the spinal cord. It is positioned ventral to the cerebrum and rostral to the spinal cord, forming the central axis of the brain. Anatomically, the brainstem is composed of the mesencephalon (midbrain), pons, and medulla oblongata. It is closely associated dorsally with the cerebellum and is traversed by numerous ascending and descending neural pathways.

During embryonic development, the brainstem arises primarily from the mesencephalon and rhombencephalon. The mesencephalon remains undivided and gives rise to the midbrain, whereas the rhombencephalon subdivides into the metencephalon and myelencephalon. The metencephalon forms the pons and cerebellum, while the myelencephalon develops into the medulla oblongata.

  • The mesencephalon constitutes the rostral part of the brainstem and surrounds the mesencephalic aqueduct. It contains the cerebral peduncles ventrally and the tectum dorsally, including the rostral and caudal colliculi, which are involved respectively in visual and auditory reflex pathways. The midbrain also contains nuclei associated with cranial nerves III and IV, reticular formation nuclei, and numerous ascending sensory and descending motor tracts.

  • The pons forms the intermediate portion of the brainstem and lies ventral to the cerebellum. It contains transverse pontine fibers, nuclei relaying information between the cerebrum and cerebellum, and portions of the reticular formation. Several cranial nerves emerge from the pontine region, including components of cranial nerves V through VIII.

  • The medulla oblongata is the caudal continuation of the brainstem and transitions caudally into the spinal cord at the level of the foramen magnum. It contains important autonomic centers regulating respiration, cardiovascular activity, and visceral reflexes. The medulla also contains ascending and descending tracts, decussation pathways, reticular nuclei, and nuclei associated with cranial nerves VIII through XII.

  • Dorsally, the brainstem forms part of the floor of the fourth ventricle, which contains cerebrospinal fluid and communicates rostrally with the mesencephalic aqueduct and caudally with the central canal of the spinal cord. The cerebellum is attached dorsally to the brainstem through the cerebellar peduncles.

Functionally, the brainstem serves as the principal conduction pathway between the cerebrum, cerebellum, and spinal cord. It integrates sensory and motor information, contains nuclei of most cranial nerves, and regulates vital autonomic and reflex functions necessary for survival. The reticular formation distributed throughout the brainstem contributes to consciousness, arousal, muscle tone, and autonomic regulation.

Compared with humans and primates, the canine brainstem occupies a proportionally larger part of the encephalon because of the relatively less expanded cerebral hemispheres.
