Tracheal branches of inferior thyroid artery

Rami tracheales arteriae thyreoideae inferioris

Definition

Antoine Micheau

The tracheal branches of the inferior thyroid artery are small arterial branches that arise from the inferior thyroid artery and supply the trachea. The inferior thyroid artery itself is a branch of the thyrocervical trunk, which originates from the subclavian artery. As the inferior thyroid artery ascends to reach the posterior aspect of the thyroid gland, it gives off several branches, including the tracheal branches.

Anatomical Details

The tracheal branches descend to supply the inferior portion of the trachea, providing blood to the tracheal wall, including its cartilaginous rings, mucosa, and surrounding connective tissue. These branches anastomose with:

  • Tracheal branches from the superior thyroid artery

  • Tracheal branches from the bronchial arteries

  • Branches from the contralateral inferior thyroid artery

This rich anastomotic network ensures a robust blood supply to the trachea and is clinically significant during thyroid surgery, tracheostomy, and tracheal resection procedures.

Clinical Significance

  • During thyroidectomy, awareness of the tracheal branches is important to avoid devascularization of the trachea, which could impair tracheal healing.

  • In tracheal resection and reconstruction, the segmental blood supply provided by these branches determines the safe length of tracheal resection, as excessive resection may compromise the blood supply to the remaining tracheal segments.

  • The tracheal branches also have relevance in tracheostomy, where inadvertent injury may lead to hemorrhage.

Course and Origin

The inferior thyroid artery typically arises from the thyrocervical trunk at the level of the C7 vertebra, ascends along the anterior scalene muscle, and then curves medially posterior to the carotid sheath to reach the thyroid gland. The tracheal branches are given off as the artery courses near the tracheoesophageal groove, along with esophageal, pharyngeal, and inferior laryngeal branches.
