The basal interventricular septum refers to the most superior (basal) portion of the interventricular septum, located near the atrioventricular junction and immediately beneath the aortic and tricuspid valves. 

The basal septum is composed of both membranous and muscular components. 

The basal interventricular septu may be involved in different pathologies:

  • The basal interventricular septum in hypertrophic cardiomyopathy (HCM) is characterized by marked asymmetric hypertrophy, most commonly involving the basal (subaortic) septum. This region often demonstrates significant thickening, which can protrude into the left ventricular outflow tract (LVOT) and contribute to dynamic obstruction. 

  • Replacement fibrosis, as detected by late gadolinium enhancement on cardiac MRI, is often distributed in the basal septal region various cardiomyopathies, and its extent is associated with adverse outcomes
